On further assessment, you find that his blood pressure is elevated, and his blood glucose levels are also high. His urine output is low, and he complains of a burning sensation while urinating. His skin is warm and dry to touch, and he has a mild rash on his chest.
As a nursing student, you are required to identify the priority problem for Mr. Williams and establish a care objective. It is important to consider his multiple age-related health conditions and the current signs and symptoms.
