3. Monthly employee wellness
consultations on-site at TechCorp

arrangement include specialist
treatment, surgical procedures,

service that Dr Anjali Mehta is not
clinically qualified or licensed to
provide.
4.3 Services will be delivered primarily
at TechCorp India Pvt Ltd's Mumbai
premises on a monthly basis. Dr Anjali

to consult by telephone or secure

5. INFORMED CONSENT, CLINICAL
INDEPENDENCE, AND MEDICAL RECORDS

employee patient's informed consent
before initiating any examination,
treatment, or diagnostic activity.
Employee patients retain the right to
decline any examination or treatment.
Informed consent requirements under

6. FEE STRUCTURE AND PAYMENT
6.1 The annual retainer fee is INR 8,00,000
per year, billed quarterly at INR
2,00,000 per quarter.
6.2 Invoices are issued on the first day
of each calendar quarter. Payment is
due within 30 days of invoice receipt
by bank transfer.
6.3 Payment does not guarantee any specific
clinical outcome. The retainer covers
the provision of professional medical
services as described in Section 4.
6.4 Late payments outstanding beyond 7
days of the due date following written
notice from Dr Anjali Mehta will accrue
interest at 12% per annum. Dr Anjali
Mehta may suspend non-emergency
scheduled services if payment remains
outstanding for more than 14 days
after formal written notice, subject
to her ethical and legal obligation
to address any genuine medical
emergency regardless of payment status.
7. EMPLOYEE PATIENT RIGHTS AND OBLIGATIONS
7.1 Employee patients have the right to
receive clinically competent and
dignified care, the right to complete

and the right to request a referral

8. EMERGENCY CARE AND REFERRALS

emergency care when an employee

In any life-threatening situation
employees should immediately call
emergency services.
8.2 Dr Anjali Mehta will refer employee
patients to appropriate specialists
